- Property, business or farm operation information (if applicable)

Why Do I Need Documentation?

Why Can’t You Access My SGI Information?
Willow Insurance is an independent brokerage offering select SGI services through partnership agreements.
We may not always have access to letters, notices, or correspondence that SGI sends directly to clients.
Please bring any related documents, notices, or letters with you to your appointment so we can assist you as effectively as possible. The most important items we need to help you are:
- Government-issued photo identification
